Why Hormone Panels Alone Fall Short

Perimenopause is often assessed primarily through hormone levels — FSH, estradiol — but these fluctuate significantly during the transition itself, sometimes day to day, making a single hormone panel a genuinely poor snapshot of where someone actually is in the process. Symptom pattern and cycle history, tracked over time, are often more clinically informative than any single hormone reading during this specific window.

The Metabolic Shift That Often Gets Missed

The decline in estrogen during perimenopause is associated with measurable shifts in metabolic health — changes in fat distribution (a tendency toward more visceral fat), insulin sensitivity, and lipid profiles — that are well documented in the research but often not proactively screened for until symptoms are already pronounced. Tracking metabolic markers (fasting glucose, lipid panel, waist circumference) through this transition, rather than only after menopause is complete, gives a more useful early picture.

Bone Density Deserves Earlier Attention

Bone loss accelerates notably in the years around the final menstrual period, and a meaningful proportion of lifetime bone density loss in women occurs in this specific window rather than spread evenly across midlife — yet bone density scanning (DEXA) is often not initiated until well after menopause under standard screening guidelines. Discussing earlier baseline bone density testing with a physician, particularly for women with additional risk factors, can be a reasonable and underused step.

Cardiovascular Risk Changes Too

Cardiovascular disease risk rises measurably after menopause, and the perimenopausal transition itself is associated with changes in blood pressure and lipid markers that can predate a formal menopause diagnosis. A fuller biomarker picture through this transition — not just hormones, but metabolic, bone, and cardiovascular markers together — gives both the individual and their physician a much more complete basis for decisions than hormone levels in isolation, and any hormonal treatment decisions should be made directly with a licensed physician familiar with the individual's full history.
