Most of the recent, better-controlled human research has focused on time-restricted eating specifically, which is worth separating from the more extreme fasting protocols when reading headlines.

What Controlled Trials Show

Randomised trials of time-restricted eating in humans have generally found modest improvements in markers like insulin sensitivity, blood pressure, and some lipid parameters over periods of weeks to months — improvements that in several trials appear comparable to what's achieved through calorie restriction alone, raising the open question of how much benefit comes from the timing itself versus simply eating less overall in a compressed window.

The Longevity Extrapolation Problem

The idea that fasting extends lifespan comes overwhelmingly from animal models — yeast, worms, and rodents — where caloric restriction and fasting-mimicking protocols do reliably extend lifespan. Translating that into a confident human longevity claim is a real evidentiary leap; no human trial has run long enough, or with mortality as an endpoint, to support it directly. The honest summary is: metabolic marker improvements, yes, reasonably well supported; direct human lifespan extension, not established.

Individual Variation Matters

Response to time-restricted eating varies meaningfully by sex, baseline metabolic health, and lifestyle — some trials in women have shown smaller or inconsistent benefits compared to men, and there are legitimate concerns about fasting protocols and menstrual cycle or hormonal effects that deserve more research than they've received. Anyone considering a structured fasting protocol, especially alongside existing metabolic conditions or medication, should discuss it with a licensed clinician rather than adopting a generic online protocol.
