What VO2 Max Measures
VO2 max is the maximum rate at which your body can consume and use oxygen during intense exercise, expressed relative to body weight. It's a composite readout of cardiac output, lung function, blood oxygen-carrying capacity, and the muscles' ability to extract and use oxygen — which is part of why it correlates with so many downstream health outcomes rather than any single organ system.
The Mortality-Risk Evidence
Large cohort studies, including a widely cited 2018 analysis of over 120,000 patients, have consistently found low cardiorespiratory fitness to be one of the strongest predictors of all-cause mortality among common clinical risk factors — in some analyses a stronger predictor than smoking, diabetes, or established cardiovascular disease. Fitness level tends to track in a graded fashion: each fitness category above 'low' is associated with progressively lower mortality risk, with no clear ceiling effect at the elite end.
How It's Actually Tested
True VO2 max testing requires a graded exercise test with expired-gas analysis, typically performed in a lab or clinical exercise-physiology setting on a treadmill or bike. Consumer wearables estimate VO2 max using heart-rate response to submaximal exercise and resting heart rate — a useful trend indicator, but meaningfully less precise than a direct laboratory test, and estimates can vary noticeably between devices and algorithms.
What Moves the Number
VO2 max responds to training — both high-intensity interval work and sustained aerobic (Zone 2) training improve it, generally through different mechanisms (interval training tends to improve cardiac stroke volume and oxygen extraction faster; aerobic base work improves mitochondrial and capillary density over a longer timescale). It also declines with age and inactivity, which is part of why tracking it over time, rather than as a single snapshot, is the more useful signal for anyone building a picture of their own trajectory.
