Beyond Hours Slept

Sleep isn't a uniform state — it cycles through stages, broadly grouped into light sleep, deep (slow-wave) sleep, and REM sleep, in roughly 90-minute cycles across the night. Total sleep duration is the easiest thing to track, but a growing body of research suggests the proportion and quality of specific stages, particularly deep sleep, may matter as much or more for several ageing-related outcomes.

What Deep Sleep Appears to Do

Slow-wave (deep) sleep is when growth hormone secretion peaks and when the glymphatic system — the brain's waste-clearance pathway — is thought to be most active, clearing metabolic byproducts including amyloid-beta, a protein implicated in Alzheimer's disease pathology. Observational studies have linked reduced deep sleep in midlife to increased markers of later cognitive decline, though causality (does poor deep sleep cause decline, or is reduced deep sleep an early marker of the same underlying process) is still being actively studied.

REM Sleep and Emotional Regulation

REM sleep, associated with vivid dreaming and high brain activity, appears more closely tied to emotional memory processing and mood regulation. Both deep sleep and REM naturally decline in proportion with age — a well-documented feature of normal ageing, not necessarily pathology on its own — which makes tracking the trend over time more informative than any single night's reading.

What's Actually Measurable at Home

Consumer wearables estimate sleep stages using a combination of movement and heart-rate variability, which correlates reasonably well with clinical polysomnography for total sleep and broad stage proportions but is considerably less accurate at the level of individual sleep cycles. For anyone with a genuine clinical concern — suspected sleep apnea, severe insomnia, or unexplained excessive daytime fatigue — a home wearable is a useful trend tool but not a substitute for a proper sleep study ordered by a physician.
